3-notch covers several decades and pattern revisions could make measurements inconsistent. A photo of the bottom, including handle, would be more illuminating.
I've been told by a couple avid collectors that it is a 3 notch and like you said, went through several revisions, but was wanting to make sure. I won't be keeping it so wanting to make sure it's legit and a possible value.
Early 3-notch Lodges are seen with raised letter molder's marks, but not typically in the location of the B here. It's hard to tell if the roughness of the casting is a result of rust pitting or just poor quality molding. The crescent shaped area at 8 o'clock to 6 o'clock on the cooking surface would support the former. The B in the wrong place and the cooking surface just being unpolished would support it being a copy.
