A little confused about this one. No markings on the bottom. Any ideas?

I'm no expert, but

I don't think it's a SMS as it does not have the triangular "pad" where the handle connects to the pan. My SMS does not have the wide pour spouts and the handle is shaped differently as well.
 
The bottom looks somewhat Lodge-like, but the bottom of the handle doesn't, nor does anything about the lid, save the raised number. Appears well-made, though, but by whom isn't clear. I've removed the SMS? from the title because it just doesn't apply at all, and I'd rather casual readers not assume that it might.
 
By the time BSR started placing their basting element dimples in a non-random, radial pattern, the single prong handles were thinner and more tapered. These dimples are also smaller and not as densely spaced as typical BSR. Even older BSR lid handles didn't have the "collars" where the ends insert into the lid, and were in fact a bit sloppy at those points. They also tended to be positioned perpendicular to the pour spout cover ears.
